IDENTIFY PATIENT(S)

• Physician identifies patients that will benefit from collaborative nursing support

 

COMPLETE REQUISITION

• Physician to complete & eFax RN Collaborate Requisition (to include service & frequency)

 

CONFIRMATION OF ORDER

• RN Collaborate confirms requisitioned order

ARRANGEMENT & PROVISION OF NURSING SUPPORT

• RN Collaborate contacts patient to arrange date/time for provision of nursing support, as ordered

• RN Collaborate provides nursing support, as ordered, in patient's home

• Documentation noted on Patient's Chart in MedAccess or eFaxed to Physician

 

COLLABORATION

• Collaboration between Physician & RN occur with documented start/end time and plan-of-care (per 14077 requirements)

 

PHYSICIAN BILLS MSP

• Physician bills MSP for 14077 [$40]

 

RN COLLABORATE INVOICES PHYSICIAN

• RN Collaborate invoices Physician for 75% of Billing Code Amount [$30]

 

RN COLLABORATE DONATES 5% FOR SDoH

• RN Collaborate donates 5% of all invoiced services to local charities that support the Social Determinants of Health in the local community
